How To Prepare A Loved One For In-Home Care Services

Moving to home-based care is a big step for any family. It often comes after you notice a loved one struggling a little more than usual, whether that’s after a fall, forgetting to take their medicines, finding everyday tasks more tiring, or simply needing an extra helping hand. While having support at home can bring reassurance, it’s also natural for older relatives to worry about losing their independence or privacy.

Taking the time to prepare together can make the transition much smoother and help everyone feel more confident about the change.

Start with their needs

Before care begins, have an open conversation about what your loved one needs help with and, just as importantly, what they can still do independently. Rather than focusing on what they can no longer manage, talk about how support can make everyday life easier, whether that’s help with preparing meals, getting dressed, managing their medicines or moving safely around the home. Involving them in these conversations helps them feel heard and gives them more control over the decisions being made. This is a key part of in-home care.

Take a closer look at daily life

Sometimes it’s the little things that show where extra support is needed. Spend a few days noticing how your loved one manages their usual routine. Are they eating regular meals? Is the washing piling up? Have they stopped going out as often or missed appointments? These small changes can help build a clearer picture of the type of care that would make the biggest difference.

Making a few notes can also be useful when speaking with care providers, helping them understand routines, preferences and any areas where additional support may be needed.

Make the home safer

Preparing the home before care starts can help prevent accidents and make everyday life more comfortable. Removing trip hazards such as loose rugs, improving lighting and keeping walkways clear are all simple changes that can have a big impact.

You may also want to consider practical additions such as grab rails in the bathroom, a shower seat or moving frequently used items within easy reach. If stairs are becoming difficult, it may be worth looking at ways to reduce the number of trips upstairs during the day.

Keep important information together

Having key information in one place makes life easier for both family members and carers. Create a folder containing a list of medicines, allergies, emergency contacts, GP details, hospital letters and any important medical information.

Don’t forget to include details such as dietary requirements, mobility aids, hearing aids or dentures, as these all play a part in everyday care. Keeping everything organised means everyone knows where to find important information if it’s needed quickly.

Share responsibilities

Caring for a loved one is much easier when everyone knows how they can help. One person might organise appointments, another could collect prescriptions, while someone else keeps in touch with carers or manages finances.

Sharing responsibilities helps prevent one family member from feeling overwhelmed and ensures nothing important gets forgotten. Even a quick family catch-up every week can help everyone stay informed and address any concerns early on.

Be mindful of emotions

Starting home care isn’t just a practical change. It’s an emotional one, too. Your loved one may worry about having someone new in their home or feel anxious about losing some of their independence.

Give them time to talk about how they’re feeling and encourage them to ask questions. Listening without rushing or dismissing their concerns can make a real difference. Keeping familiar routines in place wherever possible can also help the transition feel less daunting.

Keep the first week simple

The first few days are all about settling into a new routine. Try not to overload the schedule with visitors or appointments, allowing everyone time to adjust.

Sharing simple information with carers, such as favourite meals, preferred wake-up times, hobbies or daily routines, can help build trust and make care feel more personalised from the very beginning.

Review how things are going

As time goes on, your loved one’s needs may change, so it’s worth checking in regularly to see how things are working. Talk about what’s going well, whether any extra support is needed and if there are areas where care could be adjusted.

Staying flexible and keeping communication open between your family and care team will help ensure your loved one continues to receive the right support while remaining as comfortable and independent as possible.
